INFORMATION ABOUT ASSISTIVE TECHNOLOGY CHECKLISTS 

Practical, easy-to-access tool This Assistive Technology checklist is designed as a practical, easy to access tool for AT users and those working with people who could benefit from assistive technology.  It is provided as a quick and easy means to find categorized information about tools for typing and writing.  The two main purposes are:
1.  Assessment and consideration for assistive technology
2.  Resource information in a categorized form
Why a 'Date start' and 'Date end' on the form? Trial periods of assistive technology use for the purposes of training, practice and evaluation are essential.  It is almost impossible to determine the effectiveness of any specific equipment, system, method or intervention until the user has participated in a trial period of use.  This time period should be carefully structured with:
  • high frequency of use to provide sufficient practice and development of skill
  • precise and predetermined training
  • collection of data and information regarding the frequency of use, proficiency, competence and potential for each system tried

This is the reason that a start and end date field are provided for each piece of equipment or system.  Enter the start date as the date that the equipment / system was first introduced to the user.  The end date should indicate the date on which the trial period ended.  The end of the trial should mark a decision time when the data and observations collected during the trial will help in determining the appropriateness of the equipment or system for the user.

Quick-fill online form At the moment, this is a quick-fill form online.  The form does not gather or store any information at all.  It is designed as an online form and checklist for printing once filled out.  Information in the form will be lost once the browser window is closed.  
Report printing and record keeping for documentation Based on demand and feedback from users like yourself, we will consider adding report printing capabilities and information storage for a chronological history of AT assessment / consideration.
Printing Once you have filled out the required information, print the form using your browsers File - Print option.  Ensure that you have selected small margins or select 'fit to page' so that the form fits on the page well.  Each browser has different settings and handles forms differently so we have no way of ensuring this will print correctly.  You need to choose these settings in the print dialog box at the time of printing.
